To facilitate the expansion and coverage of Transgender through mapping and identifying hotspots in Port Elizabeth, Nelson Mandela Bay.
Strengthen collaborations with partners, community, and stakeholders to improve treatment uptake, care and support between the communities, community-based structures and identified health facilities.
Location:
Buffalo City Municipality, East London.
Key performance areas:
Conduct microplanning activities including, hotspot mapping and site profiling.
Identification of community advisory board (CAB) members to ensure active representation and engagement by target population.
Plan and implement events and campaigns, in collaboration with the outreach team, DOH and partners, according to health calendar, including community dialogues and outreach events.
Liaise with other stakeholders on healthy lifestyle programmes and collaboration.
Contribute to the evaluation of the process of community mobilisation.
Work closely with the district and sub district teams to document and respond to issues arising from the community.
Liaising with sub-partners and other community-based organisations to ensure timely interventions for human rights and gender-based violations.
Continuous engagement with the public health clinics, Department of Social Development and Police in the area to ensure good working relationships to change negative attitudes and raise awareness on the need for services for sex workers.
Support the monitoring and evaluation system to measure the coverage and impact of the community intervention.
Provide HIV Testing Services (HTS) as per DoH policy guidelines and protocol. Rapid diagnostic tests obtained from DoH.
Provide accurate health information to sex workers, especially on HIV/AIDS, PrEP, STIs, TB and contraception as well as promote healthy behaviours/habits.
Tracking and Tracing and linkage to care.
Provide and promote ART adherence support and follow-up.
Provide and promote psychological support, referral, and follow-up.
Provide accurate human rights information to sex workers.
Human Rights Defense and Support, or referral and follow-up.
Plan and facilitate the Risk Reduction Workshops and or Creative Space workshops and identify themes and topics from outreach activities.
Distribution of safer sex commodities (male condom; female condom; lubricant; IEC material).
Condom demonstrations and promoting correct and consistent condom use.
Supervision of CHWs and Team Leaders during outreach activities.
Contribute to the compilation of monthly and quarterly reports for management and donors on community engagement including compiling correct stats.
Timeously response to emails from internal and external stakeholders.
Take ownership and accountability for tasks and demonstrates effective self-management.
Follow through to ensure that quality and productivity standards of own work are consistently and accurately maintained.
Maintain a positive attitude and respond openly to feedback.
Required minimum education and training:
Grade 12.
Valid drivers license.
Fluent in English and other African languages.
Required minimum work experience:
Minimum 2 years experience in community health development and programmes.
Desirable additional education, work experience and personal abilities:
Tertiary Diploma or Degree in any relevant field including Health, Education, Public Relations, or Marketing. Certificate in HIV Management.
Drivers license.
Knowledge of Microsoft Word and Excel, electronic data collection.
Experience with community engagement and marketing new services.
Team Player: Ability to manage own time.
Ability to work without close supervision.
Adaptable, approachable, empathetic, and willing to take initiative.
Good Computer literacy.
Good communication skills (verbal and written) and able to network with various stakeholders.
Able to work independently and work as part of a multi-disciplinary team.
