Job Description

Advocates for the older persons needs and rights. Providing supportive counseling and making psychosocial assessments in oder to determine the level of care required. Facilitate new intakes/applications by performing psychosocial assessments. Assessment and developing an intervention plan in conjunction with the multi disciplinary team. Facilitate trauma debriefings with Resident and family. Assisting the older persons adjust to life in their new residency. Show an interest in the welfare and daily functioning of the residents and offer the support and counseling to help the resident overcome potential emotional, psychological and social obstacles in the home set-up.
