SA Citizen
SACSSP registered
Min 4yrs experience as a qualified Social Worker
Geriatric environment an advantage
Own transportation
Admission of new patients
Psychosocial assessments
Participation in the multi-disciplinary meetings (MDT) to ensure the best care and engagement of the patients
Drawing up of multi activity programs
Financial management process
Relationship management
Family liaison
