Job Description


OverviewMy Client a Well established NPO with a Facility / Establishment in Cape Town - which offers Care for the Aged - Assisted care as well as Childcare, is urgently recruiting for a Social Worker who will take responsibility for the provision of social work and psychosocial services to residents, both children and older persons, placed at the Facility / Establishment. Further, they are responsible for ensuring that the various residential programs are compliant with legal (Childrens Act and Older Persons Act) and departmental (as per TPA and Norms and Standards) requirements. The social worker will always adhere to the Facility / Establishment Social Work Policies, Procedures and Forms & Compliance with Norms and Standards.Education Requirements

  • University Degree / Diploma - Social worker / or similar
Competencies Required (Must Have):
  • Degree / Diploma in Social Work Registration with SACSSP
  • Must have a clean police clearance certificate and clean Form 29 name clearance on Child Protection Register.
  • At least 3-5 years experience
  • Ability to work quickly and efficiently within a pressured environment
  • Must have experience working with children and older persons
  • Excellent administration and computer skills
General Duties and Responsibilities:This position is required to:
  • Ensure that the CYCC (Child and Youth Care Centre) is compliant with all legal and departmental requirements with regards to Norms and Standards. This includes implementing, supervising, monitoring, and ensuring efficiency in the running of all programs at the CYCC, which includes:
  • Residential care programs
  • Admissions, screening of placements, reception into care, orientation
  • Therapeutic programs
  • Assessments, therapy, counselling, life skills, group work, implementation of care plan / IDP
  • Developmental programs
  • Daily stimulation program, daily structured routine, assessment, care plans / IDPs, independent living, life skills, promotion of childrens rights
  • Permanency plans, IDPs
  • Temporary safe care
  • Placements, assessment, rights, and responsibilities
  • Protection from abuse and neglect
  • Child abuse protection policy, incident reporting, behavior management, complaints procedures, promotion of childrens rights and responsibilities, education/training of carers in the prevention of child abuse and neglect
  • Assessment
  • Socio-emotional child assessments, family contact, care plans / IDPs
  • Family reunification
  • Observing and assessing parent-child interactions, reporting feedback to the external social worker, promotion of family reunification whenever possible
  • Aftercare
  • Independent living program, training / education after school, over 18yr placements

  • Clinic, hospital, children with disabilities, working with medical staff

  • ECD programs, school admissions, parent-teacher meetings, liaison with the school
  • Implement strategies needed to meet the legal requirements to ensure that The Company maintains compliance with the Norms and Standards
AdmissionsChild And Youth Care Centre (CYCC)
  • Respond to external social workers requests for placement by forwarding application forms.
  • Screen all applications (e.g., Visiting the child) and consult with other relevant role players (care workers, therapists, management) about the possibility of admission.
  • Liaise with external social worker about the application for admission - and offer or decline placement.
  • Ensure that all admissions fall within the organisations TPA with DSD, as well as ensuring that the childs needs can be met by the organisations programs.
  • Prepare the staff and assist with the admission of each child ensuring compliance with Norms and Standards as per orientation and admissions programs
  • Regularly liaise with all external social workers on:
  • the progress of intervention with the family
  • permanency planning
  • placement options
  • feedback on well-being and progress of child, etc.
  • Set up and invite the necessary role players to multi-disciplinary meetings (MDM) to discuss the care plans and permanency plans of each child - should be done every 3 months for children in temporary safe care, and every 6 months for children with finalised CCI orders.
  • Develop care plans and IDPs for all children in consultation with all the necessary role players (carers, OT, physio, external social workers, parents, children themselves, school, etc), and ensure action plans are implemented to meet the childs identified developmental needs and goals
  • Regularly liaise with schools regarding the childrens attendance, performance, behavior, peer interaction, and academic progress.
  • Attendance at parent-teacher meetings as needed.
  • Hold Monthly house meetings with staff caring for the children to evaluate and address the care plan, well-being, health and social needs.
  • Monitoring court orders, ensuring all children are legally placed, monitoring CCI orders and Section 159s.
  • Provide therapeutic intervention with children as needed.
  • Provide counselling session to parents as needed.
  • Complete attendance registers and progress reports on all sessions.
  • Refer timeously to other professionals as needed.
  • Render needed group work therapy sessions according to the Norms and Standards and TPA. Screen and assess children for suitability to participate in the group work sessions.
  • Complete needed attendance registers and progress reports on all sessions.
  • Implement therapeutic, recreational, developmental, and residential programs.
  • Ensure that the staff implementing the programs are trained and equipped to implement the programs.
  • Monitor programs and adjust according to TPA and Norms and Standards.
  • Ensure compliance with monitoring reports (attendance registers, progress reports, etc.).
Older Persons Programs
  • Provide social work services to the Elderly placed at the Establishment Home / House
  • Screening, assessment, and appropriate placement planning of all new admissions
  • Assist with all residential care enquiries, viewings & tours.
  • Process all Admission Application forms for the elderly.
  • Consult with Multi- disciplinary team re: admissions
  • Assist nursing staff with admissions to ensure residents are admitted with dignity and compassion.
  • Utilize assessments tools in line with Department of Social Development (WICHT & DQ 98, screening of files)
  • Liaising with external social workers/networks for permanency placement or referrals of older persons.
  • If assessments are required outside of service area- ensure the ability to liaise with networks, completing home visits on his/her behalf with the necessary paperwork.
  • Comply with regulations (Norms and Standards) according to the Older Persons Act 13 of 2006.
  • Attend residents meetings, Elderly Forum meeting, workshops, conferences
  • Implement strategies needed to meet the legal requirements to ensure that The Establishment maintains compliance with the Norms and Standards.
  • Provide therapeutic intervention with residents as needed.
  • Provide counselling session to families as needed.
  • Debriefing and bereavement counselling
  • Complete attendance registers and progress reports on all sessions.
  • Refer timeously to other professionals as needed.
  • Manage Reviews of DQ98s for each individual resident as per departmental requirements
  • Case management, filing, administration, maintaining regular and up to date records and process notes on each elderly file., especially incident reports and psychosocial notes.
  • Assist with financial affairs, i.e. Sassa, Home Affairs, IEC Voting registrations
  • Participate in quality assurance multi-disciplinary team meetings.
  • Complete a social history and psychosocial assessment for each resident that identifies social, emotional, and psychological needs.
  • Participate in the development of a written multi-disciplinary plan of care for each resident that identifies the psychosocial needs / issues of the resident, the goals to be accomplished for those needs / issues, and the appropriate social worker interventions.
  • Provide clinical interventions to address catastrophic events that occur during the residents stay in the facility.
  • Coordinate the resident discharge planning process and make referrals for appropriate home care services prior to the residents return to the community.
  • Monthly debtors meetings. (Mediation between residents family and house management on financial & social matters)
  • Facilitate family meetings
  • Render support groups for residents / family members / significant others as appropriate to their needs.
  • Screen and assess residents and families for suitability to participate in the group work sessions.
  • Complete needed attendance registers and progress reports on all sessions.
Community Work
  • Provide regular reports as needed to fundraise and assist with fundraising initiatives.
  • Provide regular pictures and stories to fundraising on residents s success stories and needs.
Administration
  • General office admin and recording of correspondence - telephone calls, faxes, emails.
  • Keep accurate records of all contact with external social workers, doctors, etc.
  • Open files for children and maintain complete, accurate, neat and detailed records.
  • Complete daily attendance registers, admissions registers, Establishment House Register Books, etc for all new admissions.
  • Attendance at all necessary meetings - with the DSD, external social workers, schools, in-house meetings, etc.
  • Act as mediator between management and carers, OT, Physio, and other staff, ensuring that important messages / notices are communicated to staff, as well as bringing matters to managements attention from the carers and staff this must be done in conjunction with NSM
  • Review facility policies and procedures as part of the facilitys multi-disciplinary team to assure compliance with governmental regulations.
  • Participate in reviewing and setting policies concerning resident care and quality of life.
  • Participate in developing facility social work policies.
  • Maintain monthly statistics (Admissions, discharges, deaths and therapeutic sessions).
  • Reports
  • Compile, write and submit quarterly reports to DSD, ensuring that evidence of programs is kept on file.
  • Write reports as needed by external social workers and other parties on the progress of residents and court reports.
  • Write reports as needed by fundraising and management.
  • Reports for referrals as needed
  • Compile Social Work Assessment Reports
  • Complete departmental applications for subsidies
