Faircape Health Tokai Estate is a Sub Acute Care and Rehabilitation Centre which provides restorative care and rehabilitation for patients recovering from a variety of medical conditions that include Neurological, Orthopaedic, Surgical and Respite. At Tokai Healthcare, the focus is mainly on acute conditions that immediately affect patients' rehabilitation progress.Our world-class care aims to bridge the gap between hospital and home. Our Multidisciplinary Team (MDT) of leading healthcare professionals administers care in the most therapeutic environment possible and supports healing outcomes in all aspects.Faircape Health Tokai is looking for a qualified Social Worker to coordinate and manage the discharge planning process at the Subacute facility.If you are a passionate Discharge Planner / Social Worker, join us in empowering individuals to reclaim their independence and wellness post-hospitalization.Become part of a supportive team where your skills as a social worker contribute to our mission of providing exceptional care and support to our community.Specific duties include but is not limited to the following:Discharge planning at Tokai Sub-Acute

  • Evaluate patients' medical, social, and psychological needs to develop individualised discharge plans that ensure continuity of care and optimal recovery.
  • Work closely with doctors, nurses, therapists, and other healthcare professionals to coordinate and integrate discharge plans into the overall treatment strategy.
  • Coordinate and manage discharge planning process
  • Attend and participate in daily multidisciplinary meetings.
  • Scheduling and facilitation of family meetings
  • Educate patients and their families about the discharge process, including medical aid authorization, the date of discharge, post-discharge care requirements and available community resources.
  • Confirm arrangements for post-discharge services are in place such as home based care, healthcare admission and transportation.
  • Advocate for the best interest of the patient.
  • Report suspicions of abuse or neglect according to the guidelines set out in the Older Persons Act 13 of 2006.
  • Provide feedback and communication to the appropriate MDT's.
  • Participate in quality improvement initiatives aimed at enhancing the discharge planning process, reducing readmission rates, and improving patient outcomes.
  • Provide health education/promotion written materials or presentations to residents/families
Requirements:Qualifications
  • Bachelor's Degree in Social Work
Experience and Knowledge
  • Proven experience working in a similar role
  • Experience in a hospital case management discharge planning or related medical field will be beneficial
  • Knowledge pertaining to external private and government frail cares and home care services will be beneficial
  • Knowledge of the Older Person Act

  • Computer literate
  • Clinical Assessment Skills: Ability to perform thorough assessments of patients' medical, psychological, and social needs to develop effective discharge plans.
  • Excellent communication skills (verbal and written) to effectively interact with patients, families, and the healthcare team.
  • Excellent organisational and time management skills
  • Time Management: Strong organisational skills to handle multiple cases simultaneously and ensure timely discharge planning and follow-up.
  • Problem-Solving Skills: Ability to identify potential discharge barriers and develop creative solutions to address them.
  • Demonstrate respect for the individuality of the patients and families
  • Ability to establish and maintain collaborative and effective working relationships
  • Demonstrates analytical and critical thinking abilities with proactive decision making and negotiation skills
  • Attention to Detail: A meticulous approach to ensure that all aspects of the discharge plan are addressed and nothing is overlooked.
  • Proactive Approach: Initiative to anticipate patient needs and potential discharge obstacles, taking proactive steps to address them.

Working Hours - Normal working days are Monday to Friday, 07h00 to 17h00 with a 45-minute lunch break and 15-minute tea break.The salary on offer from R 25 000 cost the company.
